The standard MenuBar (File, Edit, View, Project, Build, Debug, etc)
disappeared in my Visual Studio 2003 installation. It comes back when I go
Full Screen. I've tried repairing the installation, uninstall/reinstall, but

Have you right-clicked a toolbar, gone into Customize, and then used the
Reset... button on the Toolbars tab?

Yes, I tried that. In the "Customize" dialog, I can see the "Standard"
toolbar option, but I cannot check/uncheck the checkbox beside it. Resetting
it has no effect.

Since I last wrote, my development machine died. Apparently, reinstalling
from scratch fixes this problem. :)
