flash (macromedia) : Survey in Flash

I'd like to create a survey in Flash using multiple screens and then pass the results of the various questions to someone via e-mail (probably ASP).

Anyone have any suggestions on how to get started with and accomplish this?

First, you need to learn how to control variables inside Flash.
Then learn how to use the different components (radio buttons, check boxes,
etc.)
(From Flash's main menu)
Help > Using Flash > Using Components

If you will be getting text/string input from users, you can use textfields
or textfield component. Using Flash UI components is different from using
just textfields (Input textfield or Dynamic Textfield), so you may want to
check which one is you really need and the one that you can use easily.

Using Textfields (From Flash's main menu):
Help > Using Flash > Working with Text

Using Flash UI Components:
There's this textfield component in Flash Components Set 5. Help in using
it can be accessed through the Reference Panel once you have it installed.

Then, learn how you can pass these variables from Flash to ASP (or any other
middleware):
