One day I spent hours searching through Macromedia's site, more endless hours
in the computer manual section of several bookstores, and even more hours
searching for online tutorials that provide instruction on how to create flash
forms. Finally I paid a developer hundreds of dollars to create a simple form
that only took 30 minutes max. to complete. Hoping to help others avoid this
long search and expense, I wanted to leave a note on this forum to share for
anyone who wants to create a flash form without the expense and time involved
in learning all the code required to make an online email form. A great online
software company called coffeecup.com offers a Flash Form Builder for $34.00.
You can download a trial version for free (with a watermark). Here is the

There are several free tutorials online for creating an online flash contact
form using PHP or ASP as your server side language. The following link goes
over both. There is also a second tutorial on the same subject. Good luck.
