How critical is it that SP3a get applied to client
workstations? What are the ramifications of applying SP3a
only to the servers?

It is not critical.

What are the ramifications of applying SP3a only to the servers?

There is not problems in applying the SP3a. It is always recommended to
install Service packs in test server first and
after a weeks time you can go for production.As well as it is always
recommended to install SP's .

As Hari mentioned, I wont say its "critical" to install service packs
simultaenously in clients too.If it is, then it would be certainly mentioned
in the accompanying readme file.

There are some situations, like developing lots of DTS packages, in which
it's helpful if the clients are also in sync.
